Middle School Health Services

Beginning with the 2011/2012 school year all students are required to have 2 doses of the varicella vaccine (or history of the disease).

Grade 6 - Physical exams due - This is a state mandate. Report cards will not be issued to students with outstanding physical exams.

Grade 7 - Dental exams due. School dental exams will be done in March.

New for the 2011/2012 school year - all 7th grade students are required to have an updated dose of Tdap (if 5 years have passed since the last tetanus vaccination) and one dose of meningococcal conjugate vaccine (MCV).

If your child receives up-dated immunizations during a doctor's visit please submit of copy of his/her immunization record to the health room.

ATHLETIC PHYSICALS are required for participation in all school sports. Forms are available in the front office and the nurse's office. Complete all sections of the physical form carefully; incomplete physicals cannot be accepted. Please turn completed physicals into the nurse's office as soon as possible.

Any student who wishes to participate in a subsequent school sport must complete a re-certification form (beige form). This form is necessary to review any changes in the health status of the athlete. Forms are available in the school office or the nurse's office.
