Physical examinations are required for students entering school for the first time (kindergarten or first grade), grade 6, and grade 11. The form can be downloaded from the file below. PLEASE NOTE: this exam form CANNOT be submitted for a PIAA Sport Physical Exam. The PIAA Sport Physical Exam form can be obtained in the Middle School Offices, High School Office, High School Nurse's Office, or from the Athletic Training office.

Students with a chronic medical condition should have on file with the school nurse a plan to manage their care during the school day. Below are files to download for selected medical conditions. Please complete and submit these forms to the school nurse.

All Penn-Trafford High School Students are required to perform 30 hours of community service.

When you have completed your 30 hours of service, please submit the Community Service Form to the Guidance Department.
